Each CareMinders® Home Care location is locally owned and independently operated by a member of your own community.  Each owner considers it both a privilege and a responsibility to render the highest quality of skilled and non-medical home care services to their clients.   At CareMinders®  Home Care, our goal is to exceed the expectations of clients and their families on a daily basis, after all . . .

We offer a wide range of both long and short term assistance to our clients, regardless of age. You or your loved one may have a temporary or permanent disability or illness, or may just be in need of someone to assist with a variety of services.   Examples of the skilled and non-medical services we provide include the following:

  

 Nursing Services*

These skilled services are provided to clients by professional nurses on a full/part time or per visit basis under the care and direction of a physician.  We monitor the delivery of these services on a regular basis and continuously strive to improve the outcome with a medically specific goal in mind.  These services may include:

  • Monitoring overall health status and condition for physician response as needed
  • Medication administration, set-up and monitoring
  • Vital signs monitoring and response
  • Dressing changes, skilled procedures and other technical interventions 
  • Wound Care
  • Blood work or other lab procedures for reports to the physician
  • Infusion Therapy
  • Cardiac Rehabilitation
  • Diabetic Care
  • Dietary Regimen
  • Insulin Administration
  • Catheters
  • Ostomies
  • Colostomies
  • Tube Feeding
  • Ventilator Maintenance
  • Many other skilled services that are customized to meet the needs of our clients 

 Companion Care Services*

These services seek to assist clients to become more fully engaged in life.  Our companions become trusted friends and assist clients in socialization and every day inclusion in the joys of living.  They actively participate in activities with clients to make their time more enjoyable.  These services may include:

  • Providing companionship, socialization, friendship and support
  • Meal planning, preparation and serving in accordance with dietary requirements 
  • Grocery shopping, prescription pick-up and errand services
  • Transporting clients to shopping, social events, doctor and other appointments
  • Walking and exercise
  • Medication reminders
  • Providing respite care for other family members
  • Light housekeeping including linen changes, washing, vacuuming, mopping, and general cleaning
  • Organizing cupboards, closets and rooms
  • Caring for pets and plants
  • Ironing and mending
  • Birthday and special event reminders
  • Sunrise/sunset contact service
  • Many more services customized to meet the needs of our clients

  

  

 Personal Care Services*

These services are provided by para-professionals who assist clients with performing activities of daily living and may include:

  • Bathing
  • Dressing
  • Grooming
  • Oral hygiene
  • Incontinence and bathroom assistance
  • Ambulation
  • Mobility
  • Transferring
  • Positioning
  • Many other services that are customized to meet the needs of our clients


 

 Live-In Services

 Live in services can be offered by any level of caregiver depending on the needs of the client and the level of care required.   A live-in is recommended when the need for safety and security is of paramount importance.   Having someone available in the home around the clock provides a level of protection many clients require.  These services are always customized to meet the needs of our client.

 

Therapeutic Services*

These specialized services are provided by professionals to clients under the care and direction of a physician.  They are offered on a per visit basis and may include:

  • Physical Therapy
  • Speech Pathology
  • Occupational Therapy

Counseling Services*

The services of a medical social worker are available for assistance when counseling may be beneficial to the overall health and well-being of a client and are provided under the care and direction of a physician.  These services may include:

  • Medical Social Services
  • Dietary Counseling
  • Psychiatric Nursing
  • Advocacy and Case Management

The key to providing personalized care to clients begins with a comprehensive approach to care planning.   Our needs assessment method is unique in the industry and offers a case management approach to meet the overall needs of the client. The needs assessment includes evaluating the clinical, physical, social, functional, nutritional and psychological aspects of each client including their current health and wellness status. The assessment also includes environmental issues that may impact the outcome of improved health. We also address the difficult issues of emergency planning using our comprehensive emergency management plan. We spend considerable time in addressing prevention of injury with the client, family and caregivers.  All clients are assessed for risk of fall and caregivers will provide support to minimize the possibility of fall. We work hard at fall prevention, as injury from falls is one of the leading causes of institutionalization for the elderly.  

Our case management approach offers in-home solutions to improve the health and overall well being of the client. We plan for contingencies should problems or emergencies occur. With all the data, we are able to develop a customized service plan or plan of treatment in concert with the client, family and physician. The plan is goal specific with compatible caregivers held accountable for results toward achievable objectives.

Compatibility in caregivers is another unique approach taken by CareMinders® Home Care. Part of the assessment process includes the determination of characteristics suitable for the successful implementation of the service plan while also considering the personal preferences of clients. These characteristics are based on criteria in four categories: service needs, personality requirements, social compatibility and environmental compatibility.
